Insurances
We accept most major insurances and Medicaid including Passport. DOT-SAP services are not covered by insurance, see below for info on payment arrangements.
Out of Pocket Payments
We accepted all major debit & credit cards, as well as, Zelle & Square payments.
DOT-SAP Payment Arrangements
Payment arrangements can be made for DOT-SAP Services. Full invoice balances must be paid in full prior to receiving return to work documentation.
Cancellation & Payment Policies
All co-pays/payments are due at the time of service.
A minimum of 4 hours notice is required for appointment cancellation
Any appointments cancelled less than 4 hours prior to appointment time will incur a $20 cancellation fee
